The London Kidney Network has set up a steering group for Transition/Young Adult Care. We are looking for some peer supporters and patient representatives to join our group. We are also looking for a patient (from anywhere in the UK) who would be willing to speak at our first meeting, sharing their experience of transitioning from paediatric to young adult kidney care.
The first network meeting on Transition Care will take place on December 1st, and will involve all London adult and paediatric renal centres. Our aim is to define variations in service provision, share learning, and start the process of describing best practice in transition care.
Following this, we will be holding regular meetings of the steering group. Patient representative(s) do not have to be the same person who joined the initial network meeting, although London-based patients would be preferable in this case.
The time commitment for the initial meeting would involve delivering a short talk and participating in a discussion afterwards. If you wish to join the steering group itself you would be asked to join meetings every six weeks. All meetings would take place online and remuneration is currently being discussed.
